The Hospice Team consists of:

• A primary care and sometimes specialized care physician(s) to refer the patients to Hospice and prescribe medications.
• Registered nurses to monitor the patient's status.
• Home health aids for daily living care.
• Social Workers to help with psychological and emotional needs.
• Spiritual Coordinator to address these concerns.
• Administrative staff to coordinate billing and insurance issues.
• Volunteers to provide support in the home, as well as in the office and resource centers.
