Hospice care is primarily delivered in the home, however, those
living in adult care and nursing facilities, may be served by H&PC of VA. The patient can be followed if hospitalization is
required. Hospice uses a team approach to
care, including a number of professionals and volunteers to assure the patients
comfort and that a full spectrum of needs are being addressed. The following services are covered by H&PC of
VA appropriate to meet each patients needs:
Physician
services for the medical direction of the patients care, provided by the
patients personal physician and Medical Director of H&PC of VA.
Regular
home care visits by Registered Nurses and Licensed Practical Nurses to monitor the
patients condition, provide appropriate care and maintain patient comfort.
Home
health aide and homemaker services for personal care.
Spiritual
Support for the patient and/or loved ones.
Social work and counseling services.
Bereavement counseling for the family for up
to 13 months after the death of the loved one.
Medical
equipment such as hospital beds, wheelchairs, etc.
Medical
supplies such as bandages and catheters, etc.
Drugs for
symptom control and pain management.
Volunteer
support to assist the patient and loved ones.
Physical,
speech and occupational therapy.
Dietary
counseling.
